Mental Health Day Program Supporting Your Transition to Home
Continued step down support as you return to your home and community
The Transition To Home Day Program supports people moving from hospital care back into everyday life.
Our program provides structured support during the post-discharge period, helping you stay connected to care while adjusting to life at home.
Delivered at Fullarton Clinic in Melbourne, this mental health day program forms part of our broader outpatient care approach, supporting continuity as you transition home.
It is also suitable for some newly referred day patients who would benefit from ongoing clinical support while living in the community.
Ongoing support after hospital discharge plays an important role in maintaining stability, supporting recovery, and reducing the risk of readmission.
Evidence-based follow-up approaches, including low-intensity supports, have been shown to improve outcomes for people transitioning back into the community.
A supportive step back into daily life.
