Outpatient Referrals

Inpatient Addiction and Mental Health Services
Referrals for Mental Health Programs needs to made to a Psychiatrist with admitting rights to Fullarton Clinic
To start the process, download the form, select a Psychiatrist from our list of doctors, complete the patient details, and submit the form online.
Addiction Recovery Program
Any health professional with a Medicare provider number can refer directly to our intake team. Patients can also self refer and our intake team will manage the process.
You are still welcome to complete the referral form, including your patient’s history, and our admissions team will follow up promptly.

Next Steps After Referral
Referrals are typically reviewed within 1 business days.
Intake staff will notify you of the outcome and follow up with your patient as appropriate. Where no psychiatrist is available to accept a referral, we will inform you promptly.
A discharge summary will be sent electronically within 48 hours of patient discharge.
