An Opportunity for Continuous Support & Connection

Our Day Program teaches you how to best manage your mental health and wellbeing at home in your day to day life

The Transition to Home Day Program operates from 9am to 4pm, Monday to Friday.

Depending on your needs, you may attend as many days as clinically appropriate each week. The program is delivered in a structured, supportive environment and includes:

  • Skills for resilience and stress management
  • Psychoeducation
  • Support with emotional regulation
  • Individual check-ins where clinically appropriate
  • Nursing input where required
  • Group therapy and therapeutic activities, including gentle exercise, yoga and art therapy
  • Morning tea and lunch

Referral Pathway

How to Get a Referral

If you have recently been an inpatient at Fullarton Clinic, a Day Program referral is required.

If you are newly referred, you will generally need a referral from your GP or psychiatrist. Our intake team can guide you through the next steps.

Funding

The Transition to Home Day Program may be funded through eligible private health insurance, WorkCover, TAC or self-funding options.

We recommend checking with your health fund to confirm your cover for hospital-based psychiatric services. Our team can also assist you in understanding your eligibility and next steps.
